As a courtesy, we file claims directly with your insurance provider. Please ensure we have your most up-to-date insurance information at the time of your visit. You are responsible for any co-pays, deductibles, or services not covered by your insurance plan.
If we are not in-network with your insurance, we may still be able to see you as an out-of-network provider. Please call ahead to discuss self-pay rates and reimbursement options.
For patients without insurance or those who prefer to pay out-of-pocket, we offer transparent self-pay pricing and payment plans upon request.
Payment of co-pays is required at the time of service. Patients are responsible for any deductible amounts as determined by their insurance provider.
