Billing & Payment Information

Insurance Claims

As a courtesy, we file claims directly with your insurance provider. Please ensure we have your most up-to-date insurance information at the time of your visit. You are responsible for any co-pays, deductibles, or services not covered by your insurance plan.

Out-of-Network Patients

If we are not in-network with your insurance, we may still be able to see you as an out-of-network provider. Please call ahead to discuss self-pay rates and reimbursement options.

Self-Pay Options

For patients without insurance or those who prefer to pay out-of-pocket, we offer transparent self-pay pricing and payment plans upon request.

Co-Pays & Deductibles

Payment of co-pays is required at the time of service. Patients are responsible for any deductible amounts as determined by their insurance provider.

Prior Authorizations

Some treatments or procedures may require prior authorization. Our team will work with your insurance carrier to expedite this process and notify you of any issues.

Good Faith Estimates

Uninsured or self-pay patients are entitled to a Good Faith Estimate outlining expected charges for medical services. This is in compliance with the No Surprises Act. Please contact us prior to your appointment if you'd like to request one.
